What qualifications do Form Psychiatry providers have?

All Form Psychiatry providers are fully licensed Psychiatric Mental Health Nurse Practitioners, carefully selected for their experience, enthusiasm, and clinical expertise. You’ll always be cared for by a highly qualified, patient-focused professional.

How does billing work, and will my insurance cover this?

We bill your insurance directly. When you book, just enter your plan details and we’ll verify coverage so we can estimate your costs upfront. Most patients use in-network benefits and pay only a copay (typically between $0 and $30 per visit). We’re committed to no-surprise billing.

What is your cancellation and no-show policy?

You can cancel or reschedule at no charge when we hear from you more than 24 hours before your appointment. If you cancel with less than 24 hours’ notice or miss the visit, a late-cancellation fee applies. Insurance does not cover it. The amount is listed in our Terms of Service and the Practice Policies you receive when you join. To cancel or reschedule, use the patient portal or call or text 617-657-2947.

What if you don’t accept my insurance?

We are in-network with most major Massachusetts plans, including Blue Cross Blue Shield of MA, UnitedHealthcare/Optum, Aetna, Cigna, and Harvard Pilgrim/Tufts (Point32Health). We do not accept MassHealth. If your plan is not one we accept, we offer a flat self-pay rate for evaluations and follow-up visits. Contact us for current pricing before you book.

Do you charge for forms and paperwork?

Return-to-work letters, leave paperwork, and workplace accommodation forms are not covered by insurance and carry an administrative fee, which is collected before the completed form is released. Turnaround is about 7 business days once we have everything we need.

Is Form Psychiatry available outside Massachusetts?

Form Psychiatry currently serves patients in Massachusetts only. We’ll update this page if additional states become available.
