All Form Psychiatry providers are fully licensed Psychiatric Mental Health Nurse Practitioners, carefully selected for their experience, enthusiasm, and clinical expertise. You’ll always be cared for by a highly qualified, patient-focused professional.

We bill your insurance directly. When you book, just enter your plan details and we’ll verify coverage so we can estimate your costs upfront. Most patients use in-network benefits and pay only a copay (often around $15-$20 per session). We’re committed to no-surprise billing.

Form Psychiatry currently serves patients in Massachusetts only. We’ll update this page if additional states become available.

Providers receive bimonthly payouts for sessions completed. We handle all billing, insurance claims, and patient collections, so you can focus solely on providing care. We even offer payment protection—ensuring you’re compensated fairly in cases of insurance denials.

We manage payer credentialing and the entire revenue cycle—eligibility checks at booking, claim submission, denials/appeals, and patient billing/collections—so you can focus on care. You’ll receive reliable bimonthly payouts, with payment protection for no-shows and claim denials.

We’re currently focused on Massachusetts. If you’re licensed elsewhere and want to be considered for future opportunities, feel free to contact us, but we are not currently onboarding outside Massachusetts.
